Privacy notice

This privacy notice applies to the public website at Divine Care Services. It does not replace the practice's Notice of Privacy Practices for clinical records, billing records, or other Protected Health Information (PHI) handled by the office or approved clinical systems.

What this site collects

Website forms collect only the non-clinical contact and routing information you choose to submit, such as name, phone number, email address, preferred contact method, patient status, preferred location, callback window, general inquiry type, organization name, or insurance carrier/plan name. Some business-facing forms may include tightly scoped non-clinical notes.

What this site does not collect

Public website forms are not designed for PHI and should not be used for clinical details. Do not submit symptoms, diagnoses, medications, date of birth, Social Security number, medical record number, insurance member ID, clinical questions, uploads, appointment reasons, or emergency information through this website.

How form submissions are handled

In the current Cloudflare Pages production workflow, public forms post to /api/leads. The endpoint filters disallowed field names, requires consent, and sends a non-PHI email notification to the office at info@mydivinecareservices.com. The website does not currently claim a production lead database or patient portal record for these submissions.

A website inquiry is not an appointment, a clinical record, or an emergency contact path. The front desk reviews inquiries during clinic hours. For clinical questions, scheduling details, records, or urgent needs, call the office.

Email and service providers

Form notifications are sent through the approved website email workflow and delivered to the practice's Microsoft 365 / Outlook mailbox. The current sending provider is Resend through a Cloudflare Pages Function. Hosting, email delivery, DNS, and security providers may process the minimum technical information needed to operate the website and deliver the request.

Analytics

The current code supports lightweight first-party event tracking, such as route, page, component, and selection names. These events are intended to be anonymous and non-PHI. The website does not currently use advertising pixels, cross-site tracking cookies, or marketing retargeting tags.

Your choices

You can choose not to submit a website form and call the office instead. You can also ask the office not to use a website inquiry for follow-up. Do not use ordinary email or public website forms for medical details; call the office for the appropriate clinical communication path.
